A major bug has been found in ALL versions of Fallout 4 after taking on the Minuteman settlement quest to Monsignor Plaza.
Nothing appears to fix the bug so all you can do is wait for the patch to be released.
All you can do is avoid the Monsignor Plaza is just east of the CIT Ruins.
